Sarge Biltz's Restaurant, in Lafayette, Indiana would be a worthy addition to this list. Owned by a WW2 vet (an Army cook), who originally did most of the cooking, it's "signature" offering was a 32 ounce Porterhouse steak, with a 16-ounce baked potato. (I never ate there, but my uncle, who lived in Lafayette, did.)
