Hands down, Jean Claude van Damme playing a Cajun in HARD TARGET (was that the title?). I guess someone figured that Belgian, Cajun, whatever, a one French accent is the same as another.
Dick Van Dyke's winning personality almost makes up for it, but his Cockney accent in 'Mary Poppins' is really bad (although I never noticed that as a kid).

Sterling Hayden's swedish accent in TERROR IN A TEXAS TOWN
Humphrey Bogart as a mexican (!) in VIRGINIA CITY
James Cagney's southern accent in A LION IS IN THE STREETS
I know it's a tv show rather than a movie, but there's got to be room for Kyra Sedgewick's southern accent on "The Closer". Thenk yew.
