My least favorite: "pop" used as a positive verb. Causes unnecessary grotesqueries, as in, "Oooh, that color shirt really makes your blue eyes POP!"
I'm still upset when "impact" is used as a verb; and don't get me started on "incent" or "incentify."
The phrase popular now that irks me to no end is "It is what it is." I guarantee I've heard it five or six times in the last couple of days.
Oh, really?
Whatever.

"It is what is is" is in the top five, and I agree that it's long since lost its luster. I'd add "what goes around comes around."
It is what it is has been around for 40 year, I'd guess. It still has a place when expressing acceptance of something for itself, not for what it may mean.
I wonder if John Scalzi knows about "whatever" being the list topper, as his blog is named that.
I dislike the interior designer-speak term 'piece', as in "I found a wonderful piece for the living room." Yeah. Piece of what?
Trust me, Scalzi knows.
